can be download on the date of accepted roster announcement. After meeting and confirming the thesis advisor, students must start to return the consent form to Department Office 10 days after the date of accepted roster announcement

Other relevant information
Applicants should input referee's email address in the online application system, then the upload link for the recommendation form will be automatically sent to the referee. The recommendation form should be uploaded in time. Each link can only be used once, and uploading becomes impossible after the deadline
